It concerns over 50-year-old men and it is due to the increase in extent and volume of the prostatic gland |
| Symptomatology: |
1 Strangury 2 Pollakiuria (frequent stymulus to urination) 3 Aching pubis and perineum (sense of overpubic and perineal heaviness) 4 Nycturia (the patient often gets up at night to urinate) 5 Trouble urinating |
| Diagnosis: |
1 PSA 2 Lower abdomen ultrasound scanning 3 Uroflowmeter 4 Evaluation of the post-urinating residue. |
| Medical therapy: |
1 Alpha blocking medicines 2 Inhibitory 5 alpha reductase |
| Surgery Therapy: |
1 TURP resection by endoscopy of the prostatic tissue, by a loop which cuts and coagulates by means of the power that runs through it 2 Transvesical prostatic adenomectomy 3 Holmium laser: it grants incision and removal of the prostatic lobes. Results are similar to the ones obtained by TURP, but here there is the advantage of poor bleeding and short catheterism (48 hours at most). The disadvantage is represented by the appearance of irritative syndrome, since this tecnique goes deep, conveying heat to the prostatic capsule (this disadvantage, anyway, can be eliminated by the skill of the surgeon) 4 KTP laser (Potassium, Tytanil, Phosphate): it grants a selective photoselective vaporization of the prostate (PVP). The advantages consist in a decreased bleeding, a catheterism during less than 30 hours, a preservation of ejaculatiion and resumption of work within 7-10 days. The irritative syndrome is more likely to appear if the treatment concerns prostatic glands of remarkable extent which need greater power. 5 Thulium laser: it grants the remarkable advantage of being able to carry out both of the actions of the previous lasers with fewer side effects. This laser allows us to attack even prostate of greater extent (80-100 gr). |

The Laser Urology Centre Arezzo Italy was born thanks to the experience of Doctor Stefano Mattioli, who was the italian urologist who first introduced the use of laser in urologic surgery rooms ever since 1984. Doctor Alessandro Picinotti is another promoter of the centre and the one who means to continue such procedures through divulgation and spreading of information about the use of laser in urology.
